Youth Programs Registration Forms and Guides

87FSS Child and Youth Programs provide a comprehensive selection of care and structured, out-of-school activities for youth 9 to 18 years of age at McGuire and Dix Youth Programs.  We believe that quality programming must meet the needs of all youth in our community.

Central Registration Resource and Referral Office
Located in Building 3435 on Broidy Road at McGuire, serves as a one-stop-shop for information, availability, and registration to all Child and Youth Programs on McGuire and Dix. We also offer information of on and off-base child care and youth program options. McGuire and Dix Registrations/Enrollments for on base CYP takes place here. Fills program openings and maintains a centralized waiting list is managed through this office. Call (609) 754-3259/-3098 for an appointment to register or stop by the office for information from 8:00am to 4:30pm, Monday – Friday.

Registration for Youth Programs Center Open Recreation and Special Activities
(this is Not for Give Parent a Break Childcare, School Age Before and After Care, and Full Day Camp)

Youth 9 years to 18 years are eligible to sign up for usage of the Youth Center for Open Recreation hours.  School age children 6 to 8 years may be registered to utilize special programming when invited.  Only dependent children/youth of eligible users are able to register.

The following are eligible users:

  • Active Duty Military Personnel
  • DoD Appropriated and Non-Appropriated Fund Personnel
  • Reservist/National Guards on Active Duty Status
  • DoD Contractors
  • Retirees on a Space Available Basis

Documents Needed at the time of Registration:

○ Documentation of DEERS Enrollment is necessary for all military in the form of child’s ID or DEERS enrollment paperwork.  Civilians must provide child’s birth certificate.

○ Completion of Special Needs/Health Screening Tool Form.  Parent complete and sign the form.

○ Completion of Youth Programs Registration Form AF IMT 88.  All blocks must be completed and if not applicable, please notate with “N/A”.  See below guidance to fill out the form.

Youth Name - Last, First Name
Birth Date and Age – of the child
Male/Female – circle appropriate sex of your child
Youth Home Email - include if child has one.  If not, put “None”.
Hobbies & Interests - of youth
Sponsor Name/Rank – Last, First Name and Military Rank if on Active Duty Status.  When one parent is Active Duty military, they are always the sponsor.  When dual military family, the senior member is the sponsor.
Organization – Sponsor’s Military Unit/Place of Employment on Installation
Work Phone - Sponsor’s Work Phone Number with area code
Cell Phone – Sponsor’s
Sponsor SS# – Last 4 ONLY of Sponsor’s Social Security Number
Spouse Name/Rank – Last, First Name and Military Rank if Active Duty military.  Spouse must be listed if married and not legally divorced.  If never married or divorced, put ”N/A”.
Home Address – current local address
Work Phone - Spouse’s Work Phone Number with area code if applicable. Otherwise, put “N/A”.
Cell Phone – Spouse’s if applicable. Otherwise put “N/A”.
Home Phone – of sponsor if applicable.  If not, put “N/A”.
Emergency Contact (Other than parents) - Contact’s Name.  Needs to be someone besides the parent or guardian.  Has to live within an hour of installation.  They will only be contacted if unable to contact parents in an emergency.  If you are new to the installation, your supervisor or commander should be listed.
Emergency Phone (Same as Contact) – Phone Number(s) of Emergency Contact that you listed above block.
Photo Permission Yes/No – Circle One.  “Yes” gives us permission to photograph your child while participating in our program activities.
Sponsor Work Email – Sponsor’s complete Work E-mail address
Parent Volunteer Yes/No – If interested in helping with our programs please circle “Yes”.
Special Needs Care/Illness/Allergies/Injuries – Any special medical information needs to be included here such as Asthma, Allergies, any regular medication taken, etc.

To Register for Youth Sports Programs

In order to participate in Youth Sports Programs including Youth Sports, Sport Clinics or Classes on installation, parent needs to provide below Form.

○ Sports & Fitness Physical Examination Form – it needs to be completed by physician.

To Process Registration Forms/Documents

Please make an appointment by calling (609) 754-3259/-3098 to register.  The staffs will inform you the documents and forms you need to bring with.


If your schedule is not cooperating, you may come in to drop off the required registration forms, documents Monday through Friday (except holidays) between 8:00am and 4:30pm at Central Registration Office.  If any form(s) are found to be incomplete, parent may be required to return to Central Registration Office to complete the document properly.
